Once a common childhood illness, chickenpox has become increasingly rare in the United States thanks to a successful vaccination campaign that started in 1995. The number of cases, hospitalizations, and deaths in individuals under the age of 20 has decreased by more than 97 percent since the introduction of the vaccine. While this decline in severe cases is undoubtedly a positive outcome, it has also presented a new challenge for medical professionals.

One of the challenges that doctors face in diagnosing chickenpox is the emergence of ‘breakthrough’ cases in vaccinated individuals. These cases often present with milder symptoms, such as fewer or no blisters and milder fevers. This can complicate the diagnosis process, as the disease may look different from what doctors are accustomed to seeing in unvaccinated patients.

Accurate diagnoses of chickenpox are crucial for shaping public health policies, responding to outbreaks, evaluating vaccination programs, and understanding the changing epidemiology of the disease. However, according to a report from the Minnesota Department of Health, clinical diagnoses of varicella can be unreliable, especially in vaccinated patients.

In an effort to improve diagnostic accuracy, the Minnesota Department of Health expanded its laboratory testing for chickenpox in 2016. The department offered free testing at public health labs for suspected cases of chickenpox, including those identified in medical clinics, schools, and childcare facilities. However, the results of the testing revealed that only 37 percent of patients who provided specimens tested positive for varicella-zoster.

The success of vaccination campaigns in reducing the number of chickenpox cases has led to a shift in the landscape of the disease. While this reduction is a positive development, it has also posed new challenges for healthcare providers in accurately diagnosing the disease. The emergence of ‘breakthrough’ cases in vaccinated individuals further complicates the diagnostic process.
